Our mission is to protect the health, safety, and well being of all Union County residents by providing quality public health services.
SERVICES
General sanitation information, water sampling, food inspections, health education, school and community presentation, child passenger safety program, adult and child immunizations, WIC, Prenatal Clinic, TB skin testing, AIDS education, confidential HIV testing, vital statistics, communicable disease control and investigation, poison prevention, cancer screening, family planning, blood pressure screenings, assistance with insurance for children with medical handicaps, BCMH, child development screening

ADDITIONAL INFORMATION
Need Based:
Special Needs Council - United Way funds provided to Union County residents for needs not met by insurance or other programs such as assistance for rent or utilities, food, and medication based on income as compared to expenses. No documents are required for proof, however assistance is limited to once per 12 month period, in amounts from $50 to $150 per family.

Primary Care Clinic - Mondays and Thursdays from 10 am to 5 pm by appointment only to see a physician for $20-$30 each visit or a current medicaid card. Care Source, Molina, and Anthem BC/BS Medicaid Managed Care accepted. Medical care for acute and chronic health conditions is provided including limited samples and prescriptions, EXCEPT FOR NARCOTICS AND OTHER CONTROLLED SUBSTANCES.

Special Population:
Childhood immunizations based on ability to pay on a Sliding Fee Scale on Wednesdays. Call ahead for hours and locations.

Prenatal Clinic - Union County Residents: call for an appointment. Free confirmation of pregnancy, intake and follow along appointments for prenatal care based on ability to pay using Sliding Fee Scale based on 250% of Federal Poverty or current Medicaid card. Clinics held on Tuesdays. Guidance in Medicaid application process provided.

Reproductive Health (Family Planning) Clinic - provides birth control for women age 14 to 40 years of age based on ability to pay or current Medical Card (Anthem BC/BS, Caresource, and Molin accepted.)

Breast and Cervical Cancer screening services based on age (breast 40 years or older/cervical 50 years or older) and income. Eligibility determination determined by central office in Columbus per phone, but mammograms are provided at Memorial Hospital of Union County and pap/pelvics performed at Union County Health Department.

No Requirements:
Walk-In Morning Hours, 8-9am Monday through Friday. Free blood pressure, blood sugar screening, adult immunizations at full fees, TB tests every day but Thursdays and reading 48-72 hours later, pregnancy tests, and other consults.

Documents Required for Eligibility Determination:
Most services provided on verbal report of income, and based on ability to pay. Many service provided regardless of ability to pay. $6-$20 co-pays often requested but not required for services. Call (937) 642-0801 for details on eligibility for specific services.
