| ELIGIBILITY |
| Must have a diagnosis of a developmental disability prior to the age of 22. Qualifying diagnoses include, but are not limited to: autism, cerebral palsy, muscular dystrophy, mental retardation, Down Syndrome, spina bifida, cystic fibrosis, Prader Willi Syndrome, etc. |

Supports and Services 365 days a year for Union County citizens
Documents Required:
An individual needs documentation of diagnosis of a developmental disability prior to the age of 22. Documentation can include:
Psychological Evaluation
Multi-Factored Evaluation
Medical doctor diagnosis
