| ELIGIBILITY |
The hospice benefit is available to all patients with a life expectancy of six months or less if the terminal illness (or disease) runs its normal course. The patient, of course, must choose to elect hospice, and the patient's eligibility must be certified by a physician (most often the hospice's medical director, in conjunction with the patient's personal physician).

| ADDITIONAL
INFORMATION |
Hospice is an Underutilized Benefit
• Only a small fraction of the hospice benefit is used by eligible patients. It’s our mission to bring this gift to as many eligible patients as possible.
• 90% of people are not aware of the Medicare hospice benefit
• 75% of consumers remain unaware that hospice care can be provided in their own home
• Only 36% of eligible people receive some form of hospice care
• The average length-of-stay for hospice patients (nationally) is 51 days, even though the benefit is designed for the last six months of life.
• For every $1.00 Medicare spends on hospice, another $1.52 is saved in other medical expenses
The Gift of Hospice
• Fully covered by Medicare
• Provides care wherever the patient calls home
• Offers physical, emotional and spiritual support for patients and their families
• Provides a team of physicians, nurses, social workers, chaplains, nursing assistants and other medical professionals
• Builds a personalized plan of care that meets the individual needs of each patient and family
